PARTICIPATION &
MEDIA RELEASE WAIVER

1. Participation Agreement

I/We, the undersigned, hereby grant permission for the above-named participant to take part in all Dance Solutions / Dance Solutions Youth Outreach activities, classes, workshops, performances, and events (collectively, "Activities"). I/We understand that participation in dance and movement activities involves physical exertion and carries inherent risks of injury.

2. Assumption of Risk & Release of Liability

I/We acknowledge and voluntarily assume all risks associated with participation in the Activities, including but not limited to: physical injury, illness, property damage, or other harm that may result from participation. I/We agree to release, indemnify, and hold harmless Dance Solutions, Dance Solutions Youth Outreach, its instructors, staff, volunteers, and agents from any and all claims, damages, or liabilities arising from participation in the Activities.

I/We confirm that the participant is in good physical health and capable of participating in dance and movement activities. I/We agree to notify Dance Solutions immediately of any health conditions, injuries, or limitations that may affect the participant's ability to safely participate.

3. Medical Authorization

In the event of a medical emergency, I/We authorize Dance Solutions staff to seek appropriate medical treatment for the participant if I/We cannot be reached. I/We agree to be responsible for all medical expenses incurred.

4. Media Release & Consent

I/We hereby grant Dance Solutions / Dance Solutions Youth Outreach permission to photograph, video record, and/or audio record the participant during Activities. I/We grant Dance Solutions the right to use these materials for educational, promotional, and marketing purposes, including but not limited to: website content, social media, printed materials, grant applications, and press releases.

I/We understand that no compensation will be provided for the use of these materials and that Dance Solutions retains all rights to the materials. I/We waive any right to inspect or approve the finished materials.

5. Code of Conduct

I/We agree that the participant will conduct themselves in a respectful, cooperative manner during all Activities. Dance Solutions reserves the right to dismiss any participant whose behavior is disruptive, disrespectful, or unsafe, without refund.

6. Digital Terms of Participation

For virtual or online programming, I/We agree to the following additional terms: All virtual sessions are for the enrolled participant only. Recording, screenshotting, or redistributing session content is strictly prohibited. Participants must maintain a safe, appropriate environment during virtual sessions. Dance Solutions is not responsible for technical difficulties on the participant's end.

I/We understand that virtual sessions may be recorded by Dance Solutions for quality assurance and educational purposes.

7. Governing Law

This waiver shall be governed by the laws of the State of Maryland. If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.
